A Google Forms™ add-on
TakumiForm restyles your Google Form™, gives you a snippet you can paste on any site, and lets respondents upload files without a Google account. The same Google Sheet™ keeps catching responses.
No credit card needed up front. New here? See how it works.

Two minutes, start to embedded form. File upload and quiz scoring are one more tab in the same dashboard.
What you get
Colors, fonts, button style, corner radius, your logo. Three presets if you don't want to fuss with the knobs.
One snippet, any host. Webflow, WordPress, Squarespace, Framer, plain HTML.
Edit the form on Google and the embed picks it up. No deploy every time you fix a typo.
Replace "Submit" and "Required" with whatever you want. Send respondents to your own URL after.
Google makes respondents log in to upload a file. Your embedded form doesn't. Files land in your dashboard, private to you.
Auto-grade quiz answers and show the score right after submit, with your own pass and fail messages.
Embed
<!-- TakumiForm embed -->
<div data-takumiform="your-form-id"></div>
<script src="https://takumiform.com/embed.js" async></script> The embed auto-resizes to fit its container. No login wall, even for file uploads, and no Google Sites lock-in.
Pricing
Every feature in every tier; the sizes just scale by volume, from 100 to 10,000 responses a month. Annual billing roughly halves the rate.

Sign in with Google and you have every paid feature unlocked for 7 days. No credit card needed up front. On day 8 the paid features stop unless you've subscribed. Subscribing during the trial is fine, we won't bill you until day 7.
Yes. We sit on top of your existing form, so responses keep flowing into the same Google Sheet you already use.
Yes. Google's own file upload question makes every respondent sign in to Google first. TakumiForm adds its own upload fields to your form, so anyone can attach a file with no login. You set the allowed types and size limits, and the files land in your dashboard, private to you.
One brand instead of four. Formfacade fragments into Formfacade, Formesign, Neartail, and Promptrepo, each with its own sign-up and its own bill. TakumiForm is one account, one product. We also drop the republish dance: edit your form in Google, the embed picks it up automatically.
Your forms, responses, and settings stay in your account. Nothing is deleted on unsubscribe. The forms just stop accepting new submissions until you reactivate. Pick up where you left off any time.
