What is TakumiForm?

TakumiForm is a web service for people who own Google Forms™. You sign in with Google, connect a form through our Google Forms add-on, customize how it looks (colors, fonts, layout, button labels, your logo), and get a snippet you can paste on any website to embed the styled form. You can also add file upload fields that work without a Google sign-in, and auto-grade quizzes with instant scores.

When a respondent submits, the answers go back to Google Forms on your behalf. Your existing Google Sheet™ keeps receiving responses, and any Apps Script or Sheets automations you've already wired up keep working. Nothing changes on Google's side. More people just reach your form. Uploaded files are the one exception: Google's form endpoint can't carry them, so they're stored by TakumiForm and listed next to each response in your dashboard, visible only to you.

How it works

    Install the add-on

    TakumiForm is a Google Workspace add-on you install into Google Forms. It asks for access to the one form you open it in, nothing broader.

    Connect your form

    Open the add-on from your form's Extensions menu and click Connect. The add-on sends us that form's questions so we can render it.

    Customize the look

    Colors, fonts, layout, corner radius, button style, your logo, your own submit label and thank-you message.

    Add what Forms won't do

    Turn on file upload fields so respondents can attach files without signing in to Google, and quiz scoring so they see their result the moment they submit. Both live as tabs next to the customize editor.

    Embed it anywhere

    Copy a three-line snippet onto your site. Respondents fill in the styled form, and the answers go back to Google Forms on your behalf.

What access we ask for

Signing in on this website only asks for your name and email. There are no sensitive Google scopes on the website at all: it cannot read your Drive, your Sheets, or your forms.

Form data reaches us through the TakumiForm add-on you install in Google Forms. The add-on has access to the single form it is opened in, and it sends that form's questions to your TakumiForm account. Submissions are forwarded back to Google Forms on your behalf, so responses keep landing in your existing Google Sheet through Google's own pipeline.

The name

Takumi (匠) is Japanese for an artisan or master craftsman. TakumiForm is the same idea applied to the most common form in the world: your Google Form, but finished properly.
