Getting started
TakumiForm is a Google Forms add-on plus a dashboard at takumiform.com. The add-on connects a form; the dashboard is where you customize it, add file upload fields, set up quiz scoring, grab the embed snippet, and read responses. Everything is free for 7 days from your first sign-in, with no card needed.
1. Install the add-on
Open any Google Form and go to Extensions → Add-ons → Get add-ons, search for TakumiForm, and install it. Google asks for access to the single form you open the add-on in, nothing broader. You can also install from the Google Workspace Marketplace directly.
2. Connect a form
- Open the form you want to use and click Extensions → TakumiForm → Open TakumiForm
- Click Connect to TakumiForm. A new tab opens on takumiform.com; sign in with the Google account that owns the form
- Come back to the add-on window. It updates on its own once the form is connected
The add-on publishes your form on Google as it opens (a form has to be published before it can take responses). Repeat these steps for each form you want in TakumiForm.
Signed in on the website first, before installing the add-on? Your dashboard shows the same steps and refreshes itself when the first form lands.
3. Find your way around the dashboard
Your forms are listed at takumiform.com/dashboard. Open one and you get a row of tabs:

From the account menu in the top right you’ll find Templates (save a look and reuse it on other forms) and Billing.
4. Share or embed
Two ways to put the form in front of people, both on the Share tab:
- Public link: takumiform.com/f/… works anywhere you can paste a URL
- Embed snippet: three lines of HTML for your own site. Works in WordPress, Webflow, Squarespace, Framer, and plain HTML
Responses still land in your Google Form and its linked Google Sheet, so anything you already have wired up (notifications, Apps Script, Sheets formulas) keeps working. Uploaded files are the one thing that stays in TakumiForm, listed next to each response.
5. Editing the form later
Change questions in Google Forms as usual. TakumiForm picks the edits up the next time you open the add-on, or click Sync on the dashboard. Auto-sync runs hourly.
Common questions

| What happens after the trial? | Your forms stop taking new responses until you pick a plan. Nothing is deleted. Every plan includes every feature; they differ by volume only. |
| Which Google access does TakumiForm have? | The add-on reads the one form you open it in. Signing in on the website only shares your name and email. Details on the About page. |
